Our new patient team will review your needs, verify your insurance benefits, and get back to you. We do our very best to respond within 2-3 business days. Please note that the completion of this paperwork does not guarantee that you will be onboarded as a patient.

 Please note that our wait times vary based on clinician availability and each patient’s needs. Please also note the following:

• After-school appointment times are in very high demand and cannot be guaranteed. 

• Once you become a patient, there may be an extended wait time for initial services or delays in internal referrals. Our team is working very hard to keep these wait times brief.

• Some of our clinicians are not accepting new patients at this time.
